Ogle County Health Department is pleased to announce upcoming Vaccine Clinics. These clinics are all walk-in only.  We are not taking appointments.  Parents will need to fill out and sign on both sides of the consent form for any clinic the health department is running.  The consent form is attached and there is an English and Spanish version.  For health department run clinics, if the parent wants to fill out the form and allow grandma, aunt, etc to bring the child, that is acceptable if form was filled out by parent ahead of time.   Any family may attend any of the clinics for 12 to 17 year olds, locations at the schools does not limit it to those students only.  Please note:  Rochelle Hospital flyer states parents must accompany minor children!
